Frontal sled test have demonstrated that forward facing car seats expose children to much higher neck loads and chest displacement resulting in higher injury scores. Accident registry and crash test results support the increased safety of rear facing child seats. We reviewed the literature for evidence comparing the safety and efficacy of front versus rear facing car seats. We present three case narratives of patients aged under 3 years who sustained catastrophic injuries after being involved in a road traffic accident despite being placed in an appropriately sized forward facing car seat. Legislation governing the use of rear facing car seats is variable, with some Scandinavian countries recommending their use up to the age of four, while others mandate their use only until one year of age. However, there have been some concerns about the about the design of front facing car seats versus their rear facing counterparts. Child safety car seats have played a pivotal role in reducing the mortality and morbidity associated with road traffic accidents. Road traffic accidents are one of the commonest causes of death in children. Thus, the study provided supportive evidence for national legislation, but it also put forward that the work of popularizing law should be strengthened. More than 70% of parents supported the national legislation of child restraint use, and, among them, around 70% supported enforcement and punishment. Even though only around 20% of parents were able to respond accurately to the age standard in legislation, among those who knew of the legislation, most of the parents understood that the law had enforcement measures. There were less than 50% parents who were aware of the local legislation of child restraint use. We used descriptive statistics to describe the distribution of parental awareness and attitudes towards the legislation of child restraint. In order to understand the public attitude on national legislation in these cities, we conducted a cross-sectional survey with a sample of parents/caregivers with a child aged 0–6 years and own private car from Shanghai and Shenzhen. Mandatory child restraint law has been enacted locally in Shanghai and Shenzhen, two major cities in China. Child restraint can effectively protect child passengers.
